The Institute for Safer Medication Practices Canada

The Institute for Safe Medication Practices Canada is an independent Canadian non-profit agency established for the collection and analysis of medication error reports and the development of recommendations for the enhancement of patient safety. Like its sister organization, the Institute for Safe Medication Practices in the US, ISMP Canada intends to serve as a national resource for promoting safe medication practices throughout the health care community in Canada. World Health Organization

The WHO Surgical Safety Checklist was developed after extensive consultation aiming to decrease errors and adverse events, and increase teamwork and communication in surgery. The 19-item checklist has gone on to show significant reduction in both morbidity and mortality and is now used by a majority of surgical providers around the world.
